Medical Transitions
Transitions are difficult in general, but as your child transitions from Pediatric to Adult Healthcare, there are ways to “ease the pain”. The following are some questions that will help you make an informed decision about the doctor who will take over your son or daughter’s primary care. You may have some additional questions that you feel belong on this list. Feel free to contact your local Family TIES Coordinator at 1-800-905-TIES with your suggestions.
- What insurance does your practice accept?
- What is your experience level around young adults with significant disabilities? Are you willing to learn more about my young adult’s disability?
- My son/daughter needs to see many specialists. Are you limited to certain physicians your practice can refer to?
- What hospitals are you affiliated with?
- Are there other physicians in your practice? Who covers for you when you are not in? What is your procedure if we have an emergency or illness at night or on weekends?
- My child comes with a lot of paperwork, nursing and physical forms to be filled out, lots of medical supplies and equipment that requires prior approvals. Will the extra paperwork be an issue for you or your support staff?
- Who handles the referral process in your office? May I meet them?
- My son/daughter is in a wheelchair, is your office accessible? What about the parking, is there a space for a van with a lift?
- Do you feel you are a good advocate for your patients?
- Are you familiar with medical guardianship, self-determination, medical home? Would you like me to get you some more information?
Always remember that you are your child’s best advocate, if you don’t feel that this is the right doctor, keep looking. Most importantly make sure that your young adult is comfortable with the match. Good luck!
